NHS Nottingham & Nottinghamshire Integrated Care Board (ICB) are reviewing the existing Dementia Outreach services. Dementia Outreach services provide specialist support to people with Dementia living in a care home, alongside providing support and training to care home staff.
The service works in partnership with care home staff to improve the quality of care for people with Dementia by:
- Providing assessment and specialist support with care planning and case management
- Optimising usage of medication, prioritising non-pharmacological approaches and strategies
- Reducing avoidable hospital admissions and avoidable care home transfers
- Developing the knowledge, skills and confidence of care home staff in delivering Dementia care
- Improving pathways with other related services such as physiotherapy, occupational therapy and primary care.
The two services delivering Dementia Outreach are:
- Dementia Outreach Team (Nottingham City)
- Dementia Outreach Service (Nottinghamshire County)
