I'm pleased to share the below community survey which is now in its final version. Firstly, I just want to thank you all for your time, contribution and support over the last couple of weeks. It has been great to listen to your ideas and it highlights how well we work together. Hopefully this is a sign of things to come.
Below you will find a QR code, website link and a suggested social media post for the survey. The survey will be live until Sunday 15th June.
Our aim is to gather as many responses as possible from across the South Hardwick PCN footprint, so any support with sharing the survey far and wide would be greatly appreciated!
Our SHPCN Social Prescribers will be at hand to support with data collection. If you have any upcoming events where support is required, please let me know.
It is critical the survey is accessible for those without IT access. We will ensure paper copies are accessible in SHPCN GP Practices, but please let me know if you would like paper copies posting with a return envelope. If you have capacity to support our local community with completing the survey on their behalf, even better!
