Hours and Working Pattern: 37 hours per week

Voluntary Action Rotherham (VAR) is the lead body for supporting, developing and promoting the voluntary and community sector in the Rotherham Borough. Our role is to support and develop effective action to bring about positive change in the lives of individuals and communities in the local area. We are looking to recruit further Link Workers (Social Prescribing) to join our team to work within Rotherham’s Primary Care Networks (PCNs) to complement the existing social prescribing service.
As a Link Worker – Social Prescribing, you will be responsible for providing personalised support to individuals referred by the Primary Care Network (PCN), to help and empower them to improve their health, wellbeing and independence. The successful candidates will work within a specific PCN in Rotherham and focus on specific areas of need and patient cohorts, working intensively with individuals to determine “what matters to them most”.
