The Social Prescribing Link Worker Service takes a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support. In this position, you will help individuals to improve their health, enhance their wellbeing and maintain independence. We support and enable individuals to identify and develop self-management strategies, with empowerment being at the core of any interventions.

The service supports a range of individuals at different stages of life.  One full-time post will work mainly with older people at risk of losing or reducing independence, and the other full-time post will work mainly with younger people (aged 18-64) and support with low level mental health issues and/or alcohol /substance misuse problems, however, each post may include cover working with the other group. The part-time post will be based in the Emergency Department (ED), supporting adults (18-64 and 65+) to access appropriate care pathways and reducing the burden on the emergency care department, through signposting and connecting with SPLWs based in the community.

The successful candidates will possess excellent organisational, interpersonal and IT skills, and be able to work independently, as well as part of a team, working closely and collaboratively with SPLW colleagues in the community and other health professionals. With an awareness of the relevant issues, you will also be empathetic, sensitive and able to motivate and empower others. Experience of providing Social Prescribing support would be advantageous, but is not essential, as we are willing to train people who have the right aptitude and life experience. Due to the travel requirements of the role, the ability to drive and access to a vehicle are essential (for applicants who are unable to drive due to a disability, reasonable adjustments will be considered).
