Long Term Neurological Condition Team - Bassetlaw
We provide services for adults who have a long term neurological condition, including after a stroke. Our team is also supported by specialist community matrons with knowledge of Parkinson's disease, mutiple sclerosis and epilepsy.
Our Long Term Neurological Conditions Service provides an assessment and rehabilitation service for adults who have a long term neurological condition.
Our Community Stroke Team provides rehabilitation to everyone who has had a stroke, and can see all patients for up to a year after their stroke, excluding any time spent in hospital.
The specialist neurological nurses (for multiple sclerosis, Parkinson's and epilepsy) ensure patients with these conditions and their families receive appropriate and timely management of their condition and are supported within the community. Our service is for men and women over 18 years of age.
This service is delivered mainly in the patient's own home.
