Salary: £27,041

37 hours per week

The post will be based at the offices of DDCVS at the Agricultural Business Centre, Bakewell, although travel throughout the Derbyshire Dales will be required.

The main roles of the Social Prescribing Development Worker will be:

  • To develop and deliver a Social Prescribing service in the Derbyshire Dales that will link patients referred from GP surgeries in the Derbyshire Dales Primary Care Network with appropriate services and support from the voluntary, community and non-profit sector.
  •  To inform and support the development of new and additional services that would be beneficial to patients in the Derbyshire Dales.

No specific qualifications are required. However, the successful applicant should be able to demonstrate:

  • A good understanding of health and social care services and of the local voluntary and community sector.
  •  A good understanding of the communities of the Derbyshire Dales.
  •  Excellent communication skills, and good literacy and numeracy.
  •  Good IT skills.
  •  Excellent organisational, planning, time management and problem solving skills
